An alternative to oral NSAIDs for acute musculoskeletal injuries.

چکیده
Patients with tendon and ligament injuries often see their family physician for care. Rest, ice, compression, elevation (RICE) therapy is fi rst-line treatment for these injuries. Oral NSAIDs, such as diclofenac, piroxicam, and ibuprofen reduce swelling and lead to a more rapid return to activity than RICE alone in patients with ankle sprains, and relieve pain associated with muscle strains, too. Acetaminophen provides comparable pain relief and resumption of normal activities. Help for those who can’t take oral NSAIDs Oral NSAIDs, however, are contraindicated for patients with a history of gastrointestinal bleeding and must be used cautiously in those with chronic kidney disease. Some patients can’t tolerate the adverse eff ects, which may include stomach upset, vomiting, and abdominal pain. Others may have medication interactions that prohibit use of oral NSAIDs. Numerous high-quality, randomized, double-blinded placebo-controlled trials of topical NSAIDs have been conducted in recent years, involving diclofenac—the only topical NSAID available in the United States— as well as other topical agents on the market outside of this country. A recent Cochrane review, detailed below, assessed the effi cacy of topical NSAIDs for patients with acute musculoskeletal injuries.

Topical diclofenac epolamine patch 1.3% for treatment of acute pain caused by soft tissue injury
Acute pain caused by musculoskeletal disorders is very common and has a significant negative impact on quality-of-life and societal costs. Many types of acute pain have been managed with traditional oral non-steroidal anti-inflammatory drugs (NSAIDs) and selective cyclooxygenase-2 inhibitors (coxibs). Data from prospective, randomised controlled clinical trials and postmarketing surveillance in...
متن کاملTraumeel – an emerging option to nonsteroidal anti-inflammatory drugs in the management of acute musculoskeletal injuries
Musculoskeletal injuries are on the rise. First-line management of such injuries usually employs the RICE (rest, ice, compression, and elevation) approach to limit excessive inflammation. Nonsteroidal anti-inflammatory drugs (NSAIDs) are also commonly used to limit inflammation and to control pain. Traumeel(®), a preparation with bioregulatory effects is also used to treat the symptoms associat...
متن کاملSide effects of non-steroidal anti-inflammatory drugs: The experience of patients with musculoskeletal disorders
Non-steroidal anti-inflammatory drugs (NSAIDs) are of choice for acute pain management in musculoskeletal disorders (MSDs) but they are not without side effects. The primary aim of this study was to investigate user’s experience of side effects of oral NSAIDs among patients with MSDs. Multi-stage sampling technique was used to select 296 patients with MSDs from physiotherapy out-patient clinics...

Injuries sustained by horses during racing have been considered as an unavoidable part of horse racing. Many factors may be associated with the musculoskeletal injuries of Thoroughbred race horses. This study surveyed the amounts of nonsteroidal anti-inflammatory agents (NSAIDs) in injured horse's biological system (plasma) at Kentucky racetracks from January 1, 1995 through December 31, 1996. ...
متن کاملEvolving guidelines in the use of topical nonsteroidal anti-inflammatory drugs in the treatment of osteoarthritis
BACKGROUND Nonsteroidal anti-inflammatory drugs (NSAIDs) are a standard treatment for osteoarthritis (OA), but the use of oral NSAIDs has been linked to an elevated risk for cardiovascular and gastrointestinal adverse events and renal toxicity. Topical NSAIDs are thought to afford efficacy that is comparable to oral formulations while reducing widespread systemic drug exposure, which may provid...
